We couldn’t quite find the perfect printable book report form for my middle schooler, so we created our own.
We wanted a book report form that could be used with all books. Nothing overwhelming, but detailed enough that it assured me that my child was comprehending what he was reading.
My middle schooler uses this form when he isn’t required to be creative with one of these FUN book report ideas. I usually have him just complete this simple form out diligently with each assigned reading. It works for us.
My younger son completes a simpler book report form designed for the younger readers.
There are (3) different versions, but all cover the same information.
Middle School Printable Book Report Form Includes the Following Open Ended Questions:
- Main Characters
- Summary for each chapter ( Main Events, Problems, Ideas, Etc:)
- Beginning and Ending Events
- Favorite Part of Book
- Personal Connection
